ABSTRACT: This paper describes the process of sexoanalysis, a new way to access the different layers of fantasy material in patients with gender identity disorder. In assessing about one hundred applicants for sex reassignment surgery each year, we are aware of the increasing nosological complexity of the gender identity spectrum and of our limited understanding of the etiological and other aspects of the syndrome. This process, which uses exploration of fantasy material to explore underlying gender ambivalence, is a useful addition to the diagnostic and therapeutic procedures employed with gender dysphoric patients. Since the prognosis is poor for patients who undergo sex reassignment surgery but retain unresolved gender ambivalence, the prior examination of erotic and non-erotic fantasy material may reduce the likelihood of such negative outcomes. Sexoanalysis offers an additional method for understanding and treating the spectrum of gender identity disorders.
